Job Duties: This position provides direct clubhouse activities and case management for children between 7 years of age to 14 years of age.
*Provides social service support to teens in an innovative mental health clubhouse program. Will participate in on and off site recreational activities.
*Provides a broad range of paraprofessional behavioral health and social services to clients and their families
*Will teach daily living skills to clients and will monitor/document progress toward treatment goals.
*Assists in dealing with personal and social problems
*Flexible hours - Will need to be available for evening, night, weekend rotations.
*Will provide supportive counseling to clients and families and/or serve as a liaison for social services (50% time)
*Will focus on skill development and assist with linkage to resources.
*Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients overall progress.
*Monitors client's progress towards treatment goals.
*Provides case management and referral services to clients with emergency situations
*Completion of a Bachelor's Degree in Social Work, Psychology, Mental Health, Sociology or Counseling and one year of full-time (or equivalent part-time) experience in social services delivery.
Must be 21 years of age with 3 years of driving experience. Any applicant must not have six (6) or more points against their driver's license or a suspended or revoked driver's license within the last five (5) years.
* Drug Screen required on most employees (determined by job class).
